The method I chose to analyze The Union Carbide Malfunction is the Operations and Support Hazard Analysis (O&SHA). I chose the O&SHA because it is used “for specifically assessing the safety of operations b integrally evaluating operational procedures, the system design, and the human system integration interface.” (Ericson, 2016) Specifically, the human integration interface and focus on a specific operation. In the case of the Union Carbide mishap, the specific operation that ultimately resulted in the mishap the cleaning of a pipe used to carry the toxic chemical, MIC, and was conducted by a factory worker and supervisor. (Grazia and DeGrazia, 1985, Tragedy at Bhopal, 2008) Even within this one task, there was not one specific event that ultimately guaranteed that the mishap would occur. 

My primary recommendation for correcting the malfunction was a combination of training and procedural switch. (Grazia and DeGrazia, 1985, Tragedy at Bhopal, 2008) These methods ultimately require that the culture at the company shift to focus on safety. The culture of Union Carbide was that of normalization of deviance and general lack of care for both the training and well-being of their employees. If the employees had been sufficiently trained and the working conditions and equipment well-tended, then there would be sufficient controls to prevent the mishap.
